Most of the services have some form of monthly fee including Skype. You can currently get a Skype account with unlimited calling from your computer for $2.95 per month which is pretty close to free.
A few other services of interest are Google Voice which allows you to send incoming calls to any phone you would like. A nice thing to do with that is give out your Google Voice number (free number you can choose when you sign up) and then make it one of your favorite calling numbers so all incoming calls are now free minutes on your cell phone.
